The Family Assistance Center remains open and will be available in coordination with our local community partners. Once the Center is closed, a telephone number will be available to residents seeking additional information and assistance.

Claims forms are available in person at the FAC or the Submit a Claims page on this site. Residents may bring the necessary forms to the FAC, or complete them when they arrive. Please be sure you have read the FAQ page to understand all supporting materials that must be submitted along with your claim.

Hours

The center will be open open M-F 10a-6:30p and Saturday 10a-4p while there is community need.

What should I bring?

Our goal is to assist community members as quickly as possible. Please bring the following items with you to the Assistance Center:

- Driver's license or other government-issued identification, and Social Security card

- Proof of residency within the evacuation zone (e.g. current utility bill, etc.)

- For minors residing in the household, proof of dependency (e.g. Social Security card)

- Receipts for any expenses incurred while evacuated from your home
